Theresa E. Perlak, 74, Stevens Point, died Sunday, Dec. 4, 2011, at home, while under the care of Aspirus Comfort Care and Hospice.
A memorial service will be held at 11 a.m. Thursday at Pisarski/Dzikoski Funeral Home in Stevens Point.
Visitation will be at the funeral home from 9 a.m. Thursday until the service.

Mrs. Perlak was born Sept. 10, 1937, in Custer, a daughter of the late Henry and Mary (Betro) Lorbeck. She was a graduate of Amherst High School.
She was married to Leonard Perlak on Nov. 2, 1957, at St. Mary of Mount Carmel Catholic Church in Fancher.
She had worked in childcare and also at Pete's Silver Coach in Park Ridge.
Survivors include her husband; three sons, Randy (Jayne) and Alan (Chris), both of Stevens Point, and Kevin (Dawn), Prescott, Ariz.; three daughters, Lynn Perlak, Appleton, Dawn Perlak-Flatoff, Wautoma, and Lisa Perlak, Stevens Point; two brothers, Peter Lorbeck, Custer, and Nick Lorbeck, Torrance, Calif.; three sisters, Marian (Roy) Dash, Beaverton, Ore., Delores (Roy) Fredrickson, Lomita, Calif., and Evelyn (Joe) Rekowski, Milwaukee; 13 grandchildren; and five great-grandchildren.
She was also preceded in death by five siblings, Jerry, Charlotte, Margaret, Blaise and Conrad.
