Funeral services will be held at 10 a.m. Tuesday, Dec. 10, at the Smith Funeral Home in Eau Claire.
Visitation will be at the funeral home from 4 to 7 p.m. Monday, Dec. 9, and again from 9 a.m. Tuesday until the services.
Visitation will also be held from 2:30 to 3:30 p.m. Tuesday, Dec. 10, at the Jungers-Holly Funeral Home in Amherst. Burial and military honors will be held at St. James Cemetery in Amherst following the visitation.
A memorial will be established for the education of his children.

Mr. Trzebiatowski was born Aug. 15, 1975, in Milwaukee, a son of Ronald and Ann (Flees) Trzebiatowski. He attended school in Amherst and graduated from AmherstHigh School in 1993.
He joined the U.S. Army Reserves in 1993 and went active in 1994. Jason was stationed at many locations during his military career.
He attended Post University, graduating in 2013. He was a Boy Scout for many years.
He was married to Crista Grace-Holstein Aug. 9, 2008.
Survivors include his wife; four children Jacquieline, Jason, David Jr. and Cadence; his parents, Ronald and Ann Trzebiatowski, Amherst; and two brothers, Ryan (Maggie), Harrisburg, N.C., and Brett (Julia), Junction City; two nieces, Ariel and Ava; and two nephews, Nicholas and Matthew.
He was preceded in death by his grandparents, Jack and Genevieve (Kubisiak) Trzebiatowski and Alois and Anne (Yenter) Flees.
