Samuel E. Hertz, 23, Madison, formerly of Stevens Point, died Thursday, Dec. 27, 2012.
A memorial service will be held at 11 a.m. Monday at Frame Memorial Presbyterian Church in Stevens Point.
Visitation will be at the church from 10 a.m. Monday until the service.
Memorials may be made to Northwest Parkinson’s Association.

Survivors included his father, Robert (Leslie); his mother, Tamela Blake; two sisters, Elizabeth Hertz and Sarah (Timothy) Carlson; one brother, Charles (Angela); and his grandmothers, Elaine Hertz and Bonnie Blake.
