Daniel N. Lepak, 46, Stevens Point, died Sunday, Oct. 28, 2012, at Ministry St. Joseph’s Hospital in Marshfield.
A memorial service will be held at 5 p.m. Wednesday at Pisarski/Dzikoski Funeral Home in Stevens Point, with the Rev. Corey Brookins officiating.
Visitation will be at the funeral home from 2 p.m. Wednesday until the service.

Mr. Lepak was born April 1, 1966, in Stevens Point, a son of Janet (Milanowski) Lepak and the late Robert Lepak. He was a 1984 graduate of Stevens Point Area Senior High School.
He was married to Stephany Davis on Feb. 13, 1993, at St. Casimir Catholic Church in the town of Hull.
He was a truck driver for FedEx Freight.
Survivors include his wife; one daughter, Kaylee Rae (fiancée Jacob Sabel) Lepak, Stevens Point; his mother, Stevens Point; two brothers, Michael, Stevens Point, and Wayne (Yolanda), Newport News, Va.; three sisters, Lynn (Brian) Kuentjes, Amherst, and Connie (Mark) Kaminski and Sara Jane (Troy) Richmond, both of Stevens Point; and two grandsons.
